The Surry and Stokes County Gang Grant Initiative is supported with funds from the
North Carolina Department of Juvenile Justice and Delinquency Prevention
COMMUNITY RESPONSES

The Response Page of the Surry/Stokes Gang Grant effort is intended to collect
community and social perception data from youth, citizens, community agencies and
other sources. There are specific survey links for particular groups of our community.
Survey data is anonymous (no names or other identifiers are required) and confidential.
Results of the survey submissions will be used to better understand Surry and Stokes
County and their unique characteristics. Respondents to each survey will be required to
indicate the county in which they reside or work in. For individuals that possess
knowledge about both counties, please complete a survey for each. Estimated completion
times are indicated in italics after the survey description.

Paper copies of each survey are available under Surveys within the Library Page
of our website. Project staff will are happy to provide unlimited paper copies and
coordinate collection of our survey products to interested citizens and community groups
that wish to assist data survey collection.

This survey should be completed by youth ages 10-19 in our communities. (15 Minutes)

Community Resident Survey »
Adults in our community that are 19 and older should complete this survey.
(5 Minutes)

Community Leader Survey »
Community leaders such as elected officials, law enforcement, probation, non-profit board members, governmental board members and employees, faith based and other citizens in leadership positions will complete this survey.

School Staff Perceptions »
Survey should be completed by school staff and faculty of grades K-12.

Youth Serving Agency Survey »
Staff and volunteers of youth serving agencies (governmental, non-profit and for-profit) should complete his survey.
(5 Minutes)

Community Resource Inventory »
Please submit only one form for each community agency that provides prevention and/or intervention services of any kind to adults or youth in our communities.
